Damaged Items

What to do if your order was damaged during shipment:

Please be aware you must inspect your item(s) carefully upon receipt, and that you have a maximum of 10 days from receipt to report any damaged goods. It is important that you do not install or alter any fixture until you have inspected all items and found them to be in good condition, as neither the manufacturers, UPS nor FedEx will honor any claims for damaged merchandise after it has been installed. Items must have their original packaging, with all parts present and unaltered/uninstalled for a damage claim to be valid.

 We are sorry if you received product that was damaged during shipment, and we will do our best to take care of the problem promptly. Please contact us right away by emailing us at customerservice@formplusfunction.com or by calling 1-800-264-0057 for our customer service. We may also ask you to contact UPS at 1-800-742-5877 to report the damage. UPS needs the tracking # on the box and they will arrange for a pick-up. Please return everything to the box exactly the way you received it.

We will give you the option to order a replacement or receive a full credit for the damaged goods. If you fail to notify us within 10 days of delivery of any damaged merchandise, or if the fixture has been installed, Form + Function is not responsible for the damaged product and we will not be able to service you with a replacement or refund.


What to do if you received the wrong item or the item is defective/ parts are missing:

We inspect all items that leave our warehouse, but sometimes mistakes happen. We apologize for any inconvenience and we promise to take care of the problem right away. Please email us at customerservice@formplusfunction.com   or call us at 1-800-264-0057 for our customer service.

We will give you the option to order a replacement or receive full credit. If you fail to notify us within 2 weeks of delivery of any defective merchandise, Form+Function is not responsible for the defective product and will not be able to service you with a replacement or refund.
