| What to do if your order was damaged during shipment:
We are sorry if you receive a product that has been damaged during shipment, and we will do our best to take care of the problem promptly. Please contact us right away by filling out the form below or calling 1-800-264-0057 for our customer service. We will also ask you to contact UPS at 1-800-742-5877 to report the damage. UPS needs the tracking # on the box and they will arrange for a pick-up. Please return everything to the box exactly the way you received it.
We will give you the option to order a replacement or receive a full credit for the damaged goods. If you fail to notify us within 2 weeks of delivery of any defective or damaged merchandise, Form+Function is not responsible for the defective/damaged product and we will not be able to service you with a replacement or refund.
